Health Guardians has been recognized for their quality of care and leadership by the Louisiana Performance Excellence Awards (LPEA) Program which recognizes performance excellence and leadership in both profit and non-profit Louisiana organizations in the areas of manufacturing, service, health care, education, and the public sectors.
Health Guardians of Catholic Charities is an intensive and integrated care model that strives to remove and address any and all barriers (social, economic and health-related) to receiving effective health care. It is an integrated system of intensive medical and behavioral care navigation that incorporates the many health and social services provided by Catholic Charities and its partner organizations to help these patients achieve a better quality of life.
“This honor reaffirms the high quality intervention that the Health Guardians team provides to the community,” said Seema Dave, program director for Health Guardians. “Given that this award aligns with the internationally recognized Baldrige Quality Award, it fosters our commitment to quality improvement.”
