ALPHA HOUSE ADULT DAY HEALTH CARE CENTER

Certified Nursing Assistant
Needed part-time, for supervising and caring for clients. Assisting with client ADL’s and providing them with social, educational and recreational activities. Current Louisiana C N A certification required and recent work experience within the past 12 months. Experience working with the elderly and disabledadults, a plus. High school or GED is necessary.

BARONNE STREET TRANSITIONAL HOUSING

Program Director
Needed full-time, 5 days a week on call. Manages the day to day operation of the program directs implementation of policies and procedures in compliance with agency, funding, contracts, and accreditation. Maintains effective communication with all levels supervisor, staff, state personal, advisory board, groups, clients and volunteers, manages personnel activities, program budget, completes and submits required reports. Perform other duties as directed by supervisor. 3-4 years supervisory experience, grant writing skills, ability to understand and prepare budgets, skilled in completing statistical and programmatic reports strong computer skills (preferably excel). Experience working with families with funding for homeless programs, such as ESG, SESG and PSH. Must have the ability to communicate orally and written. Flexibility and availability 24 hrs call. Bachelors Degree in Social Services or related field required, Masters Degree preferred.

COUNSELING SOLUTIONS

Clinic Practice Manager
Responsible for all charge captures, payment posting, commercial and government follow-up of claims, responsible for collections. Knowledge of CPT coding. Familiar with managed care and Government provider enrollment.  Excellent oral and written communication skills. Excellent analytical and problem solving skills. Familiarity with an electronic medical billing system. Knowledge of Microsoft Office Suite and the ability to learn and use other technology necessary to perform job functions. Ability to function independently with minimal supervision. Associate Degree in Business and 2 years experience as a Clinic Practice Manager. Five years experience as a Clinic Practice manager may be substituted for degree.

GREENWALT ADULT DAY HEALTH CARE CENTER

Program Director
Responsible for the daily operations of The Greenwalt Center, Adult Day Health Care Program in Kenner. Oversees and directs activities to ensure the delivery of a therapeutic program to meet the individual needs of our clients. Demonstrates ability to successfully build and maintain the program census through marketing efforts, community networking and special events involvement. Registered Nurse with a valid LA license. Experience with employee supervision and working with the elderly and disabled population, preferably Alzheimer's and Dementia clients.
 

JEFFERSON CARE CENTER

Case Manager
Part-time, Monday thru Fridays. Duties includes: case managing, housing assistance and advocacy. Bachelor’s Degree in Social Work with 3 years experience.

Temporary Residential Counselor
Duties include: house management, client care, documentation and reporting. Must be able to supervise and assist with meal preparation and room monitoring. Strong organizational skills and experience with residential programs a plus. High School diploma required, Bachelor’s degree preferred. Experience with diverse population preferred and willing to work as a team. Hours: Sundays thru Thursdays from 11 p.m. until 7 a.m.

Residential Counselor (Part-time & Substitute)
Performs various responsibilities with client care, house management, documentation and reporting. Supervise and assist with meal preparation and serving and room monitoring. Candidate must have strong organizational and communicative skills. Prefer experience with residential programs. High school degree required; bachelor’s degree preferred.

Social Worker
Needed part-time, Mondays thru Fridays. Case file supervision and psychological evaluations knowledge necessary. Experience working with a diverse population preferred and willing to work as a team. LCSW degree preferred.
 

PACE (Program for All-Inclusive Care for the Elderly)

Nurse Practitioner
Needed to assess and manage the healthcare of chronically ill geriatric participants in the PACE program. Bachelor’s Degree in Nursing and graduate of an accredited Nursing Practitioner school, or a Master’s Degree in Clinical Nursing and five years nursing experience, some of which must be with a geriatric population. Must have a valid Nurse Practitioner’s license in Louisiana.

Home Care Coordinator
Provide development and implementation of home care services for the PACE participants; including the coordination of all durable medical equipment. Performs home visits, oversees home health contract provides and participates in the Interdisciplinary Team. Registered with 3-5 years of experience in home health care, hospice, geriatrics, case management, or related field. Must be licensed as an RN inLouisiana and possess a valid driver’s license with reliable transportation. Position requires frequent travel in and around New Orleans area. Must have well organizational; Leadership, interpersonal relationship, team orientated, multi-task coordination, oral and written communication skills. Works under the direct supervision of the Center Director.

Intake Coordinator
Individual responsible for coordinating all enrollment activities including assistance to potential participants in the completion of all information necessary to complete the Medicaid financial eligibility process, scheduling of home visits, and gathering information on potential enrollees. First contact person for all potential enrollees in the PACE program. Obtains key information on potential enrollees and communicates accurately basic program information to the public. Maintains all intake and enrollment information in compliance with state and federal requirements. Assists families and participants in the completion of all information needed to process Medicaid financial eligibility. Degree in Marketing, Social Work, Nursing, or Communications preferred but not required.

Physical Therapist (Part-time 20 hours plus benefits)
Responsible for providing physical therapy services for program participants on acute, restorative or maintenance levels in Adult Day Health Center. Evaluates and administers physical therapy treatments to participants, evaluates effectiveness of interventions and modifies services as appropriate. Collaborate on care plans with interdisciplinary team. Develops group therapeutic exercise programs in accordancewith participant needs. Occasional in-home treatment may be required. Graduate of a physical therapy program accredited by the APTA. Licensed to practice physical therapy in Louisiana. At least one year of general physical therapy experience with geriatric population.

Social Workers
Needed for the direct social work case management services for PACE participants. Provides social services support to include participant screening, case management, counseling and referrals. Position requires individual therapy skills geared to geriatric patients, analytical thinking, and problem solving abilities. Work performed includes visits to private homes of PACE enrollees. Position requires some travel in and around New Orleans. A valid driver’s license and reliable transportation is necessary. Master’s degree from an accredited school of social work with two years experience in a health related area. Must have a GSW or LCSW license.
 

PADUA COMMUNITY HOMES

Direct Care Workers
Provide active treatment to individuals with developmental disabilities in a community home. Participate in planning for individual active treatment including community integration activities. High school Diploma and previous experience working with the developmentally disabled persons preferred. Good writing and communication skills, a must.

Program Director
Responsible to oversee the daily operations of 5 Community Homes including the supervision of community home managers and Supervised Independent Living programs in the metropolitan New Orleans Area. Perform duties such as client admissions and discharges; coordinate the development and implementation of Individual Service Plans and quarterly assessments. Ensure programs maintain compliance with state licensing regulations. Qualified applicants will have a Masters Degree in human services or related field and at least five years experience in the MR/DD arena.
 

VOYAGE HOUSE

Therapist
Full-time individual needed to facilitate groups, provide individual therapy to homeless women ages 39 and older. LCSW or LPC preferred MSW with 5 years of experience may be considered.
