The PACE program provides a comprehensive package of health services to keep elders at home.  Services are customized to the particular needs of each participant and coordinated by our team of professionals. 

The majority of our services are provided at our beautiful and unique Adult Day Health Center, in the Bywater neighborhood, which gives our participants the opportunity to socialize and get their health care needs met in one location.

The spiritual setting, a newly renovated historic church building, offers a perfect place for reflection, as well as a social environment that keeps our participants engaged in their community.

The facility is a state of the art, comfortable environment. The program offers day time entertainment and activities, meals, healthcare, dentistry and other benefits.

Eligibility

• Age 55 or older
• Certified by the state to need nursing home level care
• Able to live in the community with PACE support without jeopardizing your health or safety
• Live in Orleans, St. Bernard or Jefferson Parish

Cost

If you have Medicare and Medicaid, you will pay nothing to PACE GNO.

If you have Medicaid only, you will pay nothing to PACE GNO.

If you have Medicare only, you must pay a monthly premium.

If you have neither Medicare or Medicaid, you will pay privately.
